You may also shorten the registration process by printing out and completing the patient forms in advance of your appointment and bringing them with you to your first visit. In order to save time when you come in for your appointment, you may download and fill out the medical history form beforehand.

+ What to Expect

Being well-prepared for your appointment will ensure that your provider has all of the necessary information to provide you with the best possible care. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment.

Please arrive at least 30 minutes prior to your scheduled appointment to complete the registration process for new patients. During check-in, you may turn in your completed registration forms and work with our Care Coordinator to ensure that your information in our system is up to date. In addition to your patient forms, please bring a valid photo identification and your insurance card if applicable. Our Care Coordinator will copy these for your medical chart, help you complete any remaining paperwork, and collect the copay and/or deductible for your visit if applicable.

At your appointment time, a medical assistant will escort you to one of our private, spacious exam or procedure rooms to help complete the relevant medical information in preparation for your visit with the provider. For comfort and privacy, you may be asked to change into a medical gown if you are having a full body skin exam, or if your visit involves an area of skin typically covered by clothing.

During your visit, your dermatologist will work to determine the nature, duration, and type of skin concern to help make a diagnosis and begin a treatment plan. While some skin conditions may be treated with prescriptions or other over the counter products, others may require a procedure to diagnose or treat.

After your visit, one of our Care Coordinators will help you finalize your visit and schedule any future appointments, if necessary.

It is our goal to make your visit as comfortable and as timely as possible. We look forward to your first visit.
